When I were a lad - no seriously! There were still pennies and halfpennies in circulation with Queen Victoria and Edward VII on them (and yes it was more than two decades after their demise, thank you). I've still got a couple somewhere.
Coins are simply withdrawn when they become too worn to be used and are replaced with ones with the current monarch's head. That's been Elizabeth for 70 years, her shillings were in circulation after decimalisation (as were those of George VI) as they were the same size as the initial 5p coin and the same value, so remained legal tender until 1990.

My Dad has a jar full of old and very worn coins, going back to Queen Victoria IIRC. Some are so worn you can barely make out the monarch.

I used to do a paper round and my best find was a 1797 penny piece.  Yes, 1797. How it managed to remain in circulation that long I have no idea.
