I recently painted my flaggellants (banner wielding, self scourging, religious nutters) with placards derding my long term gaming buddy.
These placards are about 3mm square and probably took longer than the whole unit (but it made me laugh and he only spotted it about two hours into the game).
So...what's your stupidly long piece of detail work?

I remember back to my days of playing Adeptus Titanicus/Space Marine. painting up a couple of companies of space wolves, white scars and dark angels (in black) and painting all the chapter badges and unit markings. These days I really don't have that kind of patience anymore (nothing to do with my eyesight, honest)

I painted a vignette to show all the characters from "Inquisitor."
When I worked for GW and we held speed painting competitions in store, we sometimes gave the guys 1 minute to paint a 6mm space marine >:(

I use epic 40k, and there it has a firepower 8 all around with 45 cm range :p

6mm tartans.
On 10mm, probably the "GR" and crown on SYW British mitres, or maybe the lacing on samurai ; especially hard as its not sculpted on the Pendraken figures. That's what made me give up samurai in 10mm.
On 20mm; lots of divisional patches on shoulders, black bull on yellow square, or especially problematic is the US flag. Its about giving the impression rather than strict accuracy.

Screaming IsEagle patches on 15 mm US airborne are a nightmare too!

Either the 3-colour camo smocks on my 6mm Panzergrenadiers, or the nose art on my 1/600 scale B-17s and B-24s.
